NOMINATION FORM

The Champions Club Hall of Fame banquet will be held in March each year. Individuals to be honored must have graduated from Avon Park High School ten (10) years prior to his or her induction into the Champions Club Hall of Fame. The Community Service Award is excluded from this requirement. To be a finalist, a nominee must receive two-thirds (2/3) majority vote by those present. Inductees will be considered regardless of race, color, creed, gender, religion, or political affiliation.

Nominating committee is made up of board members that are appointed by the current president of the club. All nominees must qualify under the club by-laws. The rules of parliamentary procedure as comprised in "Robert's Rules of Order" shall govern the proceedings of the Association, including Hall of Fame Selection Committee meetings, subject to special rules which may be adopted.

Nomination requirements:

  1. All nominees must be respected by peers, outstanding in his/her field, and recognized by the general population as outstanding citizens.
  2. Promotes society in a way that benefits many people.
  3. Brings honor, distinction and prestige to Avon Park or Avon Park High School.
  4. Has contributed positively to society, giving back to the community.

Guidelines for a Distinguished Alumni:

  1. One who excels in their chosen field after graduation.
  2. Participated in extra-curricular activities such as a sport or club. Volunteered their time to the school.
  3. An alumni become eligible for nomination ten years after graduation.

Guidelines for an Athlete:

  1. Exceptional athletic performance such as: College Scholarship, State Championship, All-State, All-Conference, professional career.
  2. An athlete becomes eligible 10 years after graduation.

Guidelines for Faculty, Administration, Staff, Coaches:

  1. Outstanding career as a faculty, administration or staff member and must have had a direct and significant impact on students and the community.
  2. They must have at least ten years of service to the school.
  3. They are eligible five years after retirement or leaving the school.
  4. Consideration of coaches is based on their achievement in the classroom, community, and athletic fields.
  5. Coaches are eligible one year after retirement from the school.
  6. Coaches must have at least ten years of service.

Guidelines for Community Contributor:

  1. Based on outstanding contributions made to the community for the betterment of students, staff and others involved in the community.
  2. Contribution must have a minimum of 5 years of school or community involvement.
  3. Eligible at any time.

Outstanding Teams (Teams receive special recognition):

  1. Exceptional team achievements, State or National Championship, State or National record.
  2. A team becomes eligible 10 years after team achievement.
